This notice explains how information is handled through medservus.com and the website service enquiry form. This website notice covers enquiries and website operation. It does not describe the handling of patient records or replace any separate terms agreed for a service engagement.
Information you provide
The service enquiry form asks for your name, email address, the service you need and equipment or request details. Your facility or organisation and telephone number are optional. The request includes your agreement to email these details to Medservus so the team can respond.
Please share only information needed to explain your equipment requirements. Do not submit patient names, medical records, health information, passwords or other confidential information through this form. You can also contact us directly by telephone or email.
How enquiries are used and sent
Information supplied through the form is used to review your enquiry, communicate with you and discuss the requested service. Submitting the form does not confirm an appointment or emergency dispatch.
The form sends the information you enter to the configured Medservus enquiry mailbox through the website mail system. The theme does not create a separate database record containing the enquiry. Email copies may remain in mailboxes, mail-provider systems or backups. Hosting, email and security services may process information needed to operate the website and deliver messages.
Form consent relates to responding to your enquiry; it is not a newsletter subscription. Contact us if you wish to withdraw consent for further enquiry communications. Withdrawal may limit our ability to respond and does not automatically remove records that must be retained under applicable requirements.
Technical information and spam prevention
When a page or form is accessed, hosting and security systems may process technical information such as an IP address, browser information, requested URL and access time for delivery, troubleshooting and abuse prevention.
The enquiry form uses validation and a short-lived anti-abuse counter linked to a salted hash derived from the connection IP address. Its expiry is set to ten minutes; it does not contain the enquiry message. Expired technical entries may be removed later through normal WordPress cleanup.
Cookies and third-party content
WordPress uses cookies for login and administrative preferences. They are relevant when you access an account or the website administration area; an account is not needed to submit a service enquiry. You can manage or delete cookies in your browser, although blocking login cookies can prevent account features from working.
The Medservus theme itself does not add advertising or analytics tracking. Other installed services and future integrations can have different data practices. Contact us for information about any tracking or cookie services currently in use.
Links to other websites take you to services governed by their own privacy notices. If third-party embedded content is added, that provider may receive technical information or use cookies when its content loads.
Retention and security
The enquiry form does not set an automatic deletion period for email copies. Mailbox records, hosting logs and backups can have different retention arrangements. Contact us for the applicable retention information or to request review of information connected with your enquiry.
The form includes server-side validation and protections against automated abuse. No internet transmission or email service can be guaranteed completely secure. Please avoid sending sensitive information and contact us to discuss an appropriate channel if your request involves it.
Your privacy requests
To ask about information associated with your enquiry, request access or correction, ask for deletion, withdraw consent or raise a privacy concern, email info@medservus.com with the subject “Privacy request”. You may also call +1 416 878 3644 or write to our address above.
Describe the request and provide enough information to identify the relevant enquiry. Do not email identity documents or sensitive records unless an appropriate verification method has been agreed. Identity verification may be needed before information is disclosed or changed. Requests are subject to applicable legal requirements and exceptions; deletion may not extend to records that must be retained.
